Step - 2
Once you have scheduled an appointment you will receive new patient registration forms from our practice. Prior to your visit, we recommend that you fill up registration forms (Patient Information, Medical History, Dental History) on your computer or mobile at your own convenience and it takes only 5-10 minutes. There is no need for printing, scanning, or bringing any papers with you.

If you’re like most patients you appreciate being informed before arriving at the dentist. Below is an overview of what you can expect during your first visit to Eagle Family Smiles.
First, if you haven’t been to a dentist in a while, please don’t worry. You’ve made an important decision – no need to feel guilty!
Your initial appointment will consist of a Comprehensive Dental Examination, a thorough review of your Dental and Medical health history, and necessary radiographs. An individual treatment plan will be developed based on our Doctors examination and your individual needs. Based on your current Dental condition we may be able to complete a thorough cleaning of your teeth. Our New Patient Examinations are very thorough. This means that we may need to schedule your initial cleaning on another day if the examination takes longer than usual or if you need deep cleaning/gum infection therapy. Some patients may present with complex medical history or treatment needs that may require a second appointment to discuss more comprehensive treatment options.
Important – A parent or guardian must accompany all patients under the age of 18.
Please alert the office if you have any medical conditions that may be of concern prior to Dental procedures (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.)
Our office utilizes digital radiography. This state-of-the-art Digital X-ray system allows us to take the necessary radiographs in order to diagnose your Dental condition. Your previous Dentist may have taken radiographs in the past. Unless your previous Dentist used digital radiography and can forward your most recent radiographs to our office prior to your visit we will take the necessary radiographs in order to diagnose your current conditions. Most radiographs that we see from other offices do not allow us to accurately diagnose your current conditions. It is vital that we have diagnostic-quality radiographs. If we do not have the proper radiographs to properly diagnose your current dental conditions, we will take the necessary radiographs in order to formulate the necessary treatment plan to address your Dental health. Our radiographic policy is to take the necessary radiographs to accurately diagnose the possible presence of dental disease. Regardless of what your insurance will or won’t cover or what radiographs may have been taken in the past we will take what is necessary for proper diagnosis. Generally, at your first visit to our office for a Comprehensive New Patient examination your treating Dentist will determine what radiographs he will need for diagnosis.
